Once you’ve booked your massage, please download the appropriate forms to be filled out by your massage date. In order to review your paperwork without taking away from your massage time, please come a few minutes early to your first appointment.

If your pregnancy is considered high-risk, medical approval is required before proceeding with massage. A physician’s release form is included in the forms above. If you have any special concerns, please voice them before your massage.
